National Philanthropy Day Awards – Nomination Form
Selection Process and Guidelines
The National Philanthropy Day Awards are dedicated to the celebration of donors, volunteers and organizations that have chosen to give in the spirit of Philanthropy in order to make our communities healthy, vibrant and caring.
The Association of Fundraising Professionals Edmonton & Area Chapter (AFP) welcomes nominations from registered charities, societies and individuals in Northern Alberta. All nominees and award recipients are publicly honored at the AFP’s National Philanthropy Day Luncheon.
The Philanthropy Day Adjudication Committee will make a final selection for each award from among all nominations received by 6 p.m. on Friday September 10, 2010.
All categories will be awarded based on the information provided, therefore, be thoughtful, clear, and concise in answering the questions asked. If, in the judgment of the Adjudication Committee, there is insufficient information presented to reasonably adjudicate it will be assumed that the Nominee has not met the criteria.
Please note:
- It is the responsibility of the Nominating Organization to inform the Nominee they have been nominated.
- Awards will be presented at AFP Edmonton’s Philanthropy Day Awards Luncheon. It is expected that the Nominating Organization will provide event tickets and host the Nominee(s).
- Nominations are adjudicated by the Philanthropy Day Adjudication Committee, an arms-length group of community volunteers.
- Posthumous nominations are ineligible.
Nomination Guidelines
- Entries must be received by 6:00 p.m. on Friday September 10, 2010. Late entries will not be accepted.
- Only nominations completed electronically on the Edmonton AFP website will be accepted.
- Nomination information must be provided in every field.
- Information should be provided in a narrative format. Entries will be judged on the information provided, therefore, the narrative should contain complete, clear, concise answers to criteria.
- No supporting documents are required.

- Any questions with an asterix (*) are required. Completed information is necessary for invitations to the pre-event.
- Nomination forms cannot be saved and completed at a later time. Please have all necessary information before you begin. (Handy Hint: work on the answers to the long answer questions for the nomination in a Word document. When you are satisfied with your answers, cut and paste the completed information into the online form.)
